If you wish to make a referral to Community Options please call us on 02 6295 8800 or email admin@communityoptions.com.au to discuss the referral pathways with one of our coordinators.

You will be required to complete an online referral form, which will be received by the Community Options team and subsequently processed through our referral intake process.

Please note that the client’s or authorised representative’s consent for release of information to Community Options is necessary for us to be able to process the referral.
