If you wish to make a referral to Community Options please call us on 02 6295 8800 or email admin@communityoptions.com.au to discuss the referral pathways with one of our coordinators.

Then you will need to complete a referral form and submit it to Community Options by mail, e-mail, or fax.

Please note that the client’s or authorised representative’s consent for release of information to Community Options is necessary for us to be able to process the referral.
