As we have previously announced Community Options is exiting the direct provision of aged care services. We are currently undertaking the process of supporting all our Home Care Package clients to transition to new providers and we anticipate this process to be completed by the end of April. As soon as the Commonwealth appoints a provider/s to take over our current CHSP funding we will commence the process to transition all of these clients to new providers, we anticipate this process will be completed by 30 June 2025. Additionally, we are reducing our role as a provider of NDIS Support Coordination with a focus on supporting existing long-term clients and working with new clients in a targeted and outcome focussed way.
Once these processes are complete Community Options ACT will continue to provide important community support services to the ACT and region community.
In this new environment we will continue to offer independent, grassroots case management services that will build on our three decades of dedicated service to our community.
We will continue to focus on supporting at-risk individuals and families across various sectors.
