Trilogy Care expertly manages the finance and administrative aspects of your funding, allowing Community Options to focus entirely on what matters most— you and your care.
As your dedicated Care Manager, Community Options becomes your primary point of contact. We coordinate your supports, manage any changes, and ensure everything aligns with government requirements—so you don’t have to worry about the details. We’re by your side during Care Plan and Budget meetings, working closely with Trilogy Care to ensure your needs are clearly understood and fully addressed. If your circumstances change, we’ll initiate review meetings on your behalf, making transitions to your care smooth and stress-free.
