Supporting older Canberrans to access the care they need
Community Options’ Care Finder program is here to support older people who need extra help to access aged care services and don’t have family or friends who can assist.
We provide free, personalised support to help you navigate the aged care system and connect with the services you need to live safely and independently.
Care Finder is supported by funding from the ACT Public Health Network (PHN) through the Australian Government’s PHN Program.
How We Can Help
Our friendly Care Finder Coordinators can support you with:
- Registering with My Aged Care
- Understanding and preparing for assessments
- Finding and connecting with local aged care and community services
- Completing forms and understanding service agreements
- Checking in once services are in place to ensure everything is working well
Who We Support
You may be eligible for Care Finder support if you:
- Are 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people)
- Are experiencing homelessness or at risk of becoming homeless
- Are socially isolated or don’t have someone you trust to help you access aged care
- Face communication barriers such as low literacy, language or cultural differences, or limited digital skills
- Have difficulty processing information or making decisions
- Are reluctant to engage with aged care services due to past experiences or fear of institutions
- Are at immediate risk or in a situation that could lead to a crisis without support
